Plant: erect, perennial, 4"-12" tall forb with a single stem
Flower: pink to purple, 6-parted, 1 1/4" - 2 1/3" long; lip spotted and streaked with purple and yellow and with a tuft of hairs; solitary, erect; blooms May-June
Leaf:solitary, 5"-10" long, grass-like, found only on mature plants
Habitat: wet; bogs, meadows, fens
